What a Systematic Review Says About Microplastics and Your Health
A 2024 systematic review graded the evidence and reached a cautious verdict: microplastics are 'suspected' to harm reproductive, digestive, and respiratory health.
Most headlines about microplastics run ahead of the science. A 2024 systematic review in Environmental Science & Technology did the opposite: it graded the evidence formally, then stated the most it could honestly conclude. The verdict was that microplastics are “suspected” to harm human reproductive, digestive, and respiratory health, with a suggested link to colon and lung cancer.[Environ. Sci. Technol., 2024]
“Suspected” is a specific term, not a hedge. It sits on a four-step hazard scale — known, presumed, suspected, not classifiable — borrowed from the National Toxicology Program’s approach.[Environ. Sci. Technol., 2024] Understanding where the evidence actually lands is more useful than any alarming or dismissive headline.
How the review was actually done
This was a rapid systematic review using the Navigation Guide method — a transparent, predefined process built to translate environmental health science into evidence-graded conclusions.[Environ. Sci. Technol., 2024] The team searched four databases in July 2022 and again in April 2024, with no restriction on publication date.[Environ. Sci. Technol., 2024]
A rapid review trades some thoroughness for speed. Here, that meant one person screened all studies while a second checked only exclusions, and a single reviewer assessed risk of bias while another verified the calls.[Environ. Sci. Those three human cross-sectional studies covered reproductive outcomes (two) and respiratory outcomes (one), with total study populations between 40 and 80 people.[Environ. Sci. Technol., 2024] There were no human studies of digestive effects — that entire conclusion rests on animals.[Environ. Sci. Technol., 2024]
The rest of the evidence came from 28 experimental animal studies: 11 reproductive, seven respiratory, and 10 digestive.[Environ. Sci. Technol., 2024] Most were conducted in mice, most in China, and the microplastic tested was overwhelmingly polystyrene — used in 22 of the 28 animal studies.[Environ. Sci. Technol., 2024]
What the evidence supported most strongly
Two outcomes earned the review’s top “high” quality rating for the body of evidence.
The first was sperm quality. Five animal studies, all using polystyrene, reported declines in living sperm, sperm concentration, and motility, alongside increases in sperm malformation as exposure rose.[Environ. Sci. Technol., 2024] Every study pointed in the direction of harm, giving an estimated proportion of harmful effects of 1.00.[Environ. Sci. Technol., 2024]
The second was immunosuppression in the digestive tract. Two studies measured immune-system biomarkers and reported significant changes across immune cell populations, again all in the direction of harm.[Environ. Sci. Technol., 2024]
Key takeaway
Even for these two “high” quality outcomes, the conclusion was still only “suspected” — not “presumed.” The authors explicitly downgraded them “based on the sample size and number of studies.”[Environ. Sci. Technol., 2024] High-quality evidence from a handful of small studies is still a handful of small studies.
A larger set of outcomes landed at “moderate” quality and the same “suspected” conclusion: reduced ovarian follicles and altered reproductive hormones; gross and microanatomic effects on the colon and small intestine, altered cell proliferation and death, and chronic gut inflammation; and, in the airways, reduced pulmonary function, lung injury, chronic inflammation, and oxidative stress.[Environ. Where the evidence didn’t hold up
The review is at its most useful where it declines to draw a conclusion.
Birth outcomes and gestational age in humans were rated “unclassifiable” on the basis of “low” and “very low” quality evidence.[Environ. Sci. Technol., 2024] On birth weight specifically, the two human studies disagreed: one found a significant correlation between placental microplastic load and reduced birth weight, length, and head circumference; the other found no difference tied to microplastics in amniotic fluid.[Environ. Sci. Technol., 2024]
Several animal outcomes were also left unclassifiable, including litter size, testicular aging, anogenital distance, and digestive oxidative stress — mostly because the evidence was too sparse or low quality to support a conclusion.[Environ. Sci. Technol., 2024] The single human respiratory study, linking microplastic levels to chronic rhinosinusitis, was rated “very low” quality and likewise unclassifiable.[Environ. Sci. Technol., 2024]
There is also a structural limitation worth naming. The data across studies were so inconsistently reported that the team could not perform a meta-analysis at all — estimates came on different scales, used different metrics, or weren’t fully reported.[Environ. Sci. Technol., 2024] Instead they estimated the proportion of effects pointing toward harm, a cruder synthesis. And because the included studies tested only three polymer types plus tire-wear particles, in fairly uniform spherical shapes, the review can’t speak to the far greater real-world variety of microplastics.[Environ. Why “suspected” may still understate the risk
A reasonable reader might treat “suspected” as reassuring. The authors argue the opposite.
They state plainly that “given these limitations, it is likely our conclusions underestimate the true health impacts of microplastic exposure.”[Environ. Sci. Technol., 2024] Several factors push that way at once: their use of p-values to flag harmful differences tends to undercount real effects; null-result studies may never have been published; and only one study each looked at sensitive life stages or added stressors.[Environ. There’s context, too. The review notes plastic production is projected to triple by 2060, and that microplastics have already been measured in human placenta, breastmilk, and liver.[Environ. Sci. Technol., 2024] Given that ubiquity, the authors argue it is likely microplastics affect body systems this review never examined.[Environ. Sci. Technol., 2024]
Their policy stance follows from that reasoning: the need for more research “should not preclude action,” and regulators can act on limited evidence because such evidence tends to strengthen over time.[Environ. Sci. Technol., 2024]
What to actually take from this
The honest summary is narrow. A rigorous, transparent review found consistent signals of harm across three body systems, graded that evidence carefully, and stopped at “suspected” — while warning that the real picture is probably worse, not better.
For a reader, that supports a measured response rather than either panic or dismissal. The strongest direct exposure routes flagged in the review are food and water (ingestion) and air (inhalation).[Environ. Sci. Technol., 2024] Reducing avoidable ingestion — a common one being from plastic packaging in contact with food and drink — is a reasonable, low-cost hedge while the science matures. It is not a proven intervention, and this review does not test any specific mitigation.
